Rocky Balboa is een tweederangs bokser uit Philadelphia. Hij is een mislukking: hij woont in een afgedankt appartement en als bijbaantje is hij een inner van schulden. Ook probeert hij indruk te maken op de ietwat verlegen Adrian, zijn grote liefde.
Nadat hij de bokswereld heeft gedomineerd, is het Adonis Creed zowel in zijn carrière als in zijn gezinsleven voor de wind gegaan. Wanneer jeugdvriend en voormalig bokstalent Damian weer opduikt na een lange gevangenisstraf, wil die graag bewijzen dat hij een kans verdient in de ring. De confrontatie tussen de voormalige vrienden is meer dan alleen een bokspartij. Om de rekening te vereffenen moet Adonis zijn toekomst op het spel zetten om het op te nemen tegen Damian, een bokser die niets te verliezen heeft.
Adonis Creed heeft zijn beroemde vader, de wereldkampioen zwaargewicht Apollo Creed, nooit gekend doordat hij stierf voor zijn geboorte. Toch valt niet te ontkennen dat boksen in zijn bloed zit. Adonis besluit naar Philadelphia te reizen naar de plek waar de legendarische wedstrijd tussen Apollo Creed en Rocky Balboa plaatsvond. Eenmaal in de stad zoekt hij Rocky op en vraagt hem om hem te trainen. Ondanks dat Rocky uit de bokswereld is gestapt ziet hij potentieel in Adonis.
Wanneer Rocky's vriend Apollo Creed doodgeslagen wordt tijdens een gevecht met de superfitte Russische bokser Ivan Drago, geeft Rocky zichzelf hier de schuld van. Opgehitst door de arrogantie van de Rus besluit Rocky het gevecht met hem aan te gaan, dit keer echter in Rusland.
Een jonge, veelbelovende sporter ontvlucht met zijn vrouw communistisch Polen om zijn droom na te jagen: de grootste bokser uit de geschiedenis worden.
Na Apollo Creed verslagen te hebben heeft Rocky eindelijk de tijd om het eens wat rustiger aan te doen. Hij raakt bevriend met Creed en brengt meer tijd met zijn vrouw door. Wanneer hij echter verslagen wordt door de nieuweling Clubber Lang wil niemand meer iets van hem weten. Creed weet Rocky over te halen om een rematch te organiseren.
Adonis Creed staat voor zijn grootste uitdaging ooit. Dit terwijl hij persoonlijke verplichtingen heeft en moet trainen voor zijn volgende grote wedstrijd. Zijn tegenstander Viktor Drago, zoon van Ivan Drago, is geen onbekende voor zijn familie.
Rocky Balboa loopt een ernstige oogwond op tijdens het gevecht met zwaargewicht Apollo Creed en moet geopereerd worden. Hoewel de ingreep geslaagd is, adviseren de artsen niet meer te boksen. Rocky volgt hun advies op maar ontdekt al snel dat het leven buiten de ring moeilijk en hard is.

Voormalig wereldkampioen boksen George 'Iceman' Chambers belandt in de gevangenis, nadat hij wordt veroordeeld voor de verkrachting van een vrouw. Hij wil zich onmiddellijk bewijzen en hij daagt de ongeslagen gevangeniskampioen Monroe Hutchen uit voor een gevecht. Na een opstootje in de kantine worden de twee vechtersbazen gescheiden. De directie van de strafinstelling besluit Hutchen in de isoleercel te plaatsen om de gemoederen te bedaren. Dit is echter slechts een voorlopige maatregel: onder toedoen van de maffia, die actief is binnen de gevangenismuren, komt de confrontatie er toch.
Nadat gebleken is dat de Russische bokser Ivan Drago Rocky permanente hersenbeschadiging heeft toegebracht, is Rocky gedwongen om te stoppen. Wanneer hij thuiskomt komt hij er achter dat al zijn geld gestolen is. Rocky besluit een jonge bokser te gaan trainen. Hij kan echter niet op tegen de hoge salarissen die de jongen aangeboden worden door andere managers.
Het verhaal gaat over de agressieve, gewelddadige bokser Billy Hope die zich naar de top weet te werken. Wanneer de weltergewicht-bokskampioen door zijn gedrag een gevecht uitlokt, met de dood van zijn vrouw als gevolg, valt zijn persoonlijke leven echter aan diggelen.

De film volgt Harry Haft, die wordt gedreven door zijn liefde voor een vrouw om de onvoorstelbare gruwelen van de Duitse concentratiekampen te overleven. Een SS-officier dwingt Harry tot de dood te vechten tegen medegevangenen in gruwelijke gladiatorenwedstrijden. Harry ontsnapt uiteindelijk en komt naar New York. Achtervolgd door zijn herinneringen en schuldgevoel over zijn overleving, neemt hij het op tegen bokslegendes, zoals Rocky Marciano, in de hoop dat zijn naam zal worden opgemerkt en hij zijn eerste liefde weer zal vinden.
De wat oudere bokser Ramón krijgt nieuwe energie als hij de felle jonge bokser Deborah ontmoet. Door hun ongeremde passie heeft hij alleen nog maar oog voor haar.
Door financiële omstandigheden zijn Tom Riley en zijn vader John genoodzaakt te verhuizen naar een van de mindere wijken van Chicago.
